12/7/2023 EnchantaVaughn Approved 1.Behavior: Individual counseling held with Laylah at the home. QMHP-C inquired about her day. Laylah reported to QMHP-C her day was going well. QMHP-C engaged Laylah in a discussion to determine how her behavior has been since the last session. Laylah reported she has been doing good, âJust chillingâ. Mother reports Laylah struggles with building healthy relationships with others as she had a verbal altercation with her grandmother in the home. Mother reports Laylah stormed out the room when her grandmother was talking to her and crying for 20 minutes in duration. 2. Behavior: Laylah participated in the session and did not need any redirection from QMHP-C during the session. Laylah was able to discuss with QMHP-C on how she can resolve conflict without engaging in negative behavior. She became defensive as she made attempts to explain her reason for blaming others for her behaviors at times throughout the session as well with behaviors lasting 2-3 minutes in duration 1. Intervention: QMHP-C explored Laylahâs underlying issues for blaming others for her behavior. She was challenged to accept responsibility for her behaviors regardless of the consequences. QMHP-C modeled appropriate ways to employ conflict resolution techniques during several situations. QMHP-C assisted Laylah with maintaining the use of calming and coping strategies during challenging situations. QMHP-C revisited several past situations in which conflict resolution techniques were not utilized. QMHP-C instructed Laylah to talk about how she would respond to such situations at the present moment. QMHP-C assisted Laylah via play with constructing a client-tailored strategy for managing her anger. 2. Intervention: QMHP-C processed with Laylah on what causes hostile anger in her. During this time QMHP-C discussed with Laylah the ABC Model as a treatment plan to introduce to her new ways of viewing anger. QMHP-C engaged Laylah in a discussion encouraging her to start discussing with QMHP-C to identify her motivation for change and the asking her to identify her personal anger patterns. Towards the end of the session QMHP-C suggested to the mother Laylah needed a new psychological to better assist her with her anger and mood stability. 1. Response: Laylah had a nonchalant attitude during this session. She answered some questions but was hesitant about others. She appeared as if she was tired and restless. She did manage to provide adequate feedback regarding the topic being discussed as it relates to blaming others. She became defensive as she made attempts to explain her reason for blaming others. She fails to recognize her wrong doings in specific situations. She continues to struggle with resolving conflicts in a peaceful manner. She was receptive to the strategies for managing anger. She has done well with reducing the intensity of her anger outbursts. 2. Response: Laylah stated to QMHP-C that she feels as though her teachers are always showing favoritism towards others in the classroom. Laylah also slated to QMHP-C that as a consequence she has to receive detention due to her telling the teacher her needs and wants. QMHP-C Interpretation of clientâs progress towards todayâs objectives: Laylah continues to struggle with controlling her anger and implementing learned coping skills. QMHP-C Plan of Action: VACCD staff will continue to assist Laylah with building healthy relationships with her mother and others she come in contact with.
